safeSPACE™ Connect is a holistic service designed to support young people aged 10-25 through personalised guidance from qualified Youth Workers

Through Case Management, Youth Workers walk alongside each individual to define their goals and build a path to reaching them. Through the process, young people are connected with specialised agencies and community resources, ensuring they have the tools and networks needed to achieve their full potential.

How does Case Management work?

Case Management is about empowering young people to achieve the goals that matter most to them. While each young person’s journey looks different, the core focus of case management is to help build strong support networks, foster positive relationships, and increase their ability to navigate essential systems and processes – such as job applications or finding the right educational path – through goal setting and achievement.

 

Case Management isn’t counselling or therapy. Instead, it focuses on practical outcomes, leading to the development of life skills, greater resilience, improved self-esteem, and the adoption of protective behaviours. It also supports the growth of meaningful social connections and relationships, contributing to increased stability across various aspects of life.

 

PCYC’s Case Management program is driven by the young person, assisting them to identify areas where they need support and helping them create achievable goals.

Targeted Programs

We recognise that the transition to adulthood can be challenging, especially since services available for this age bracket are limited.

 

That’s why safeSPACE™ Connect offers targeted programs for young people aged 16-25, integrating life skills and goal-setting within enjoyable and engaging activities like art and sports.

 

These programs look different across our four locations and are referral-based, by a referring agent of the young person themselves.

Primary School Programs

safeSPACE™ Connect also offers facilitated programs for Year 5 and 6 children in Primary Schools.

 

Program options range from whole classes to targeted groups, with various lengths on offer.

 

These programs are designed to support young people to remain engaged in education whilst learning strategies and skills to participate more fully in society.
